Please bring your referral letter from a GP or other doctor, your Medicare card, DVA card, or Pension Card, private hospital insurance information, any relevant test results (X-rays, MRI, CT scans), and your sleep study report or CPAP download data if applicable.

If you need to cancel an appointment, please call our office during business hours and provide at least 24 hours' notice so that we can offer your appointment time to another patient.
